Both open and minimally invasive spinal fusions are commonly performed neurosurgical interventions. As new technology has developed so have opportunities to perform spinal procedures in a minimally invasive manner with similar outcomes. There are circumstances where either open and MIS approaches are clearly favored, but increasingly there is clinical equipoise about these approaches in many clinical scenarios. We will discuss the application of open and minimally invasive approaches to spinal fusion, including circumstances where each is clearly favored and where there is controversy amongst surgical approaches.
